Chronic pain self-management program is a five-week comprehensive program that aims to empower the patients to effectively self-manage their pain to improve their quality of life. It includes chronic pain education session and self-management strategies that can be practiced daily.

Weekly sessions focus on demonstrating and performing management strategies like breathing, relaxation, stretching techniques…. It is customized for chronic pain patients to address their needs in an individualized approach.

This program pathway encourages equal participation of the patients based on their current physical ability. It allows them to share their experiences, stay connected and feel supported being in their same peer group.

It encourages to develop their own action plan and develop a habit of adhering to it and help stay motivated. The patients are guided and supported throughout the session to ensure that they are empowered to self-manage their chronic pain.

How is this program conducted?

Our Pain specialists will prescreen to make sure you will benefit from this program. Then this program will be included in your treatment plan. Informative education Brochures, session plans will be provided to encourage self-participation with no hinderance.

The sessions are provided virtually in groups to participate at your convenience in your comfortable home environment. Referrals can be made from your pain care physician/family physician.
